We are an Altus Dental Dentist that serves Arlington, Belmont, Cambridge, Lexington, Medford, Somerville, Waltham, Watertown, Winchester and Woburn.

There are three Dental Plans offered by Altus Dental: Altus for One, Altus Preferred,  and Altus Preferred Plus.

Altus One Person Dental

  • Eligible Companies: A firm with one or more full time employees that maintains a membership with HSA
  • Eligible Employees: All full-time employees working a minimum of 30 hrs per week
  • Waiver: Employees covered through a spousal dental plan may be excluded. A waiver form must be completed showing proof of coverage
  • Participation Requirement: 70% of the eligible employees must be enrolled
  • Employer Contribution: The employer must contribute a minimum of 50% of the employee premium
  • New Hires: All newly hired employees must be enrolled the first of the month following 30 days of continuous employment or a time frame consistent with company policy

Altus Preferred and Altus Preferred Plus

  • New Hires: All newly hired employees must be enrolled the first of the month following 30 days of continuous employment or a time frame consistent with company policy.
  • Employer Contribution: The employer must contribute a minimum of 50% of the employee premium. 
  • Participation Requirement: 100% of the eligible employees must be enrolled
  • Waiver: Employees covered through a spousal dental plan may be excluded. A waiver form must be completed showing proof of coverage
  • Eligible Employees: All full time employees working a minimum of 30 hrs per week
  • Eligible Companies: A Massachusetts firm with 2 or more eligible employees, enrolling and maintaining a membership with HSA
